All insurance is subject to verification and eligibility prior to rendering services.  Deductibles and copays may vary depending on the requirements of your carrier.
 

Please be sure to notify the receptionist if you know you have a secondary insurance policy to avoid delays in processing to your carrier.

Some insurance carriers require pre-authorization prior to treatment in our office.  Please check with you carrier prior to treatment to avoid unnecessary liability.  

If you fail to notify us in the event your insurance changes during the course of your care, you may be liable for the cash rate associated with your visits. 

MEDICARE:  The base deductible for 2025 is $257.  If you fulfill your deductible us, you are good for the year with all other providers.

Be advised that not all deductibles are covered by your secondary carrier. 

We do our best to verify your responsibility with your respective carrier, however, the explanation of benefits you receive after your claim has been submitted will dictate your financial responsibility. 

Effective 09/01/24:  UHC requires prior-authorization for chiropractic and physical therapy related services.  This must be completed prior to treatment at our facility.

Please be advised that UHC cards that have the
Ucard  logo on the front of the card are NOT accepted.

UHC cards that show processing of claims with
Intermountain or Optum are NOT accepted.

HPN/BCBS/CIGNA patients are required to pay for their x-rays on the first visit.  If after billing on your behalf, your carrier does pay for the x-rays, you will be reimbursed accordingly.  

Humana patients:  If your insurance card has anything showing HMO, Intermountain, or Optum they are NOT accepted.
